1. About These Terms

The Health Panel Terms supplement the general Dialogue Terms of Use you agreed to when creating your account. They set out the additional conditions that apply specifically to this service. The general Terms of Use continue to govern your use of the platform.

You can access the service through the Dialogue app, website, or through a partner-branded application such as Canada Life's Consult+, or Sun Life's Lumino Health Virtual Care or my Sun Life. In these Terms, "platform" refers to any of these access points.

By purchasing the Health Panel, you confirm that you have read and agreed to these terms.

How we handle your personal information is described in our Privacy Policy, available on the platform under "Info & Legal" in your profile.

 

2. Description of Service

What the service is

The Health Panel is a preventive health screening service focused on cardiovascular and metabolic health.

The components of the service cannot be purchased or accessed separately. The package includes an intake questionnaire, blood and urine testing at an eligible laboratory, a clinical review by a Dialogue clinician, a personalized health report, and a medical appointment to discuss your results and recommendations.

Your results will be available on the platform. You may view, download, and share them as you see fit. They are released in two stages:

  • When your results are ready after your laboratory visit: A Dialogue clinician reviews your results before releasing them to you. You will receive your laboratory results along with an indication of whether each falls within the reference range (e.g. high, average, low).

  • At your medical appointment: You will receive your complete personalized health report.

What the service is not

The Health Panel is not a replacement for your regular healthcare providers. It is not meant to diagnose or treat existing health conditions and is not designed to investigate symptoms, manage chronic conditions, or address current health concerns. The service assesses specific health markers and is not a comprehensive assessment of your health.

Normal results do not mean you are free of health conditions. The Health Panel does not screen for every condition and does not replace the routine screenings or care your regular healthcare provider recommends. If you have symptoms or health concerns, please refrain from using the service and instead consult with a healthcare provider.

 

3. Who Can Use the Service

Who the service is for

To access the Health Panel, you must be of the age of majority in your province of residence, be located in Quebec or Ontario at the time of intake and your medical appointment, and have confirmed that an eligible laboratory is accessible to you before purchasing.

 

Who the service is not for

During intake, you will be asked questions to determine whether the Health Panel is appropriate for you. There are situations where it may not be suitable, including pregnancy or breastfeeding, an active health concern or symptom requiring diagnosis, investigation, or treatment, and a chronic condition under management. This list is not exhaustive. We may pause or stop the service at any point if your health profile indicates it is not appropriate for you.

 

4. Your Laboratory Visit

Your Health Panel laboratory requisition is valid only at designated eligible laboratories and cannot be used elsewhere to complete the service.

Please follow all preparation instructions provided on the platform before your visit, including any fasting, urine sample requirements, or other instructions. Not following these instructions may affect the accuracy or interpretation of your results.

Eligible laboratories are independent. Dialogue coordinates your laboratory requisition and receives your results, but the collection and processing of your samples are outside our direct control. We are not responsible for errors, delays, or failures attributable to a laboratory. If an issue arises with your sample or results, we will help you coordinate follow-up.

 

5. Payment, Receipts, Refunds, and Service Expiry

Fees

The Health Panel costs $499, payable at intake. Payment can only be made by credit card or prepaid credit card through the platform. You may not pay an eligible laboratory or clinician directly.

Payments are processed by a third-party payment processor. Dialogue is not responsible for errors or delays attributable to the payment processor.

The Health Panel is not covered by provincial health insurance plans such as RAMQ or OHIP, and cannot be submitted for reimbursement under these plans.

Coverage through private benefit plans, such as a Health Spending Account (HSA), varies by plan and is not guaranteed. If you plan to seek reimbursement, check with your insurer before purchasing.

 

Payment confirmation and insurance receipt

After payment, you will receive a payment confirmation. This is not an insurance receipt.

Your insurance receipt will be issued only after you complete your medical appointment, at the end of the service. It cannot be issued before then. If you do not complete your medical appointment, no insurance receipt will be provided. An insurance receipt does not guarantee reimbursement.

 

Refunds

You may request a full refund before completing your laboratory visit. Once your laboratory visit is complete, the service is non-refundable. If you have questions or concerns about a charge, contact us through the platform or at support@dialogue.co.

 

Service expiry

You have one year from the date of purchase to complete both your laboratory visit and medical appointment. Services not completed within this period expire without refund.

 

6. Clinical Relationship and Follow-Up

The Health Panel is a one-time screening service and does not create an ongoing care relationship or replace your regular healthcare providers.

If your results require a follow-up, a member of our care team will contact you to recommend appropriate next steps. You are responsible for responding and following up with an appropriate resource, such as Dialogue’s Primary Care program, your treating physician, or another healthcare provider, as appropriate for your needs.

The clinicians who deliver the Health Panel are licensed professionals responsible for their own clinical decisions.

 

7. Your Responsibilities

By purchasing the Health Panel, you agree to:

  • Provide accurate and complete information during intake. Incomplete or inaccurate information may affect your results or recommendations;

  • Follow all preparation instructions for your laboratory visit;

  • Attend your medical appointment and respond to follow-ups from our care team;

  • Keep your account and contact information updated so we can reach you if needed; and

  • Comply with the general Dialogue Terms of Use.
